Main criteria:

· exemplary professional expertise (attainment of local, national or international prestige that has brought honor, distinction to FU, hometown, country) = 50%

· social/civic involvement (significant contribution in FU alumni affairs) = 30%

· unblemished moral uprightness, ethical reputation, and integrity = 20%

· willingness to attend awards night in Dumaguete City

Categories:

· Agriculture

· Arts & Culture

· Sciences

· Accountancy

· Entrepreneurship

· Corporate Business Management

· Education

· Law

· Community Service (distinguished contribution to the local community in the Philippines for the last 10 years; need for testimonial by beneficiaries. Ex. Environmental advocacy)

· Government/Public Service (at least 10 years)

· FUAA Service Award (meritorious spearheading, organizing of alumni project)

· Outstanding Chapter Award (meritorious spearheading, organizing of alumni project in their respective town)

Mechanics:

1. Any alumni of Foundation University can nominate other alumni for the various categories by presenting:

o 3 copies of the nominee's resume

o 3 copies of the rationale for the nomination (why the nominee deserves to get the award) to be typed on one page short bond paper.

o 3 copies recent 2x2 photo

2. Officers of the FUAA and members of the Membership Committee are disqualified from making nominations, or being nominated, for the awards.

3. All nominations must be submitted by Tuesday, Nov. 2, 2005 to the FUAA Inc. at the Balay sa Alumni, FU North Campus, V. Locsin St., Dumaguete City. Decisions of the Committee will be finalized by Nov. 5, 2005. Awarding will be during the Kasadyaan Homecoming on Saturday, Dec. 10, 2005.
