RESIDENCE HALL RULES AND REGULATIONS ON DISCIPLINE
1. POLICY
1.1 Any regular and / or transient Hall Resident who commits an offense against the Residence Hall Management or its Personnel shall be subject to disciplinary action.
2. COVERAGE
2.1 All regular or transient Residents and employees.
3. ADMINISTRATION
3.1. Front Desk Personnel, Floor Leaders, Night Stewards and Night Matrons, Heads, Supervisors, or the Resident Hall Manager directly overseeing hall residents.
4. PROVISIONS
4.1 Admission Rules: The following rules and procedures are to be strictly observed and followed.
4.1.1 Only Nursing Students are allowed to avail of the privilege of staying in the residence hall.
4.1.2 The Resident must comply with all the enrollment requirements of Foundation University before he/she can apply for the Hall Services.
4.1.3 After complying, Resident shall fill out hall application form available at the Business Office, the Office of the Manager of Auxiliary Services or the Office of the Resident Manager, completing all the attachments required.
4.1.4 Resident shall be required to sign in the CONFORME of the Manual on Rules and Regulations, which is attached to the Hall Application Form.
4.1.5 Resident is required to fill out the forms provided for the Guardians/ Parents with their specimen signatures. In submitting the Form, the resident is required to attach the photocopies of the valid Identification cards of the guardians mentioned in the Form.
4.1.6 Hall application form submitted without the necessary and required attachments shall not be processed.
4.1.7 Resident cannot choose his or her room assignment or his or her room mate(s).
4.1.8 There shall be an Application Fee of ONE HUNDRED (Php100.00) PESOS that shall be paid at the Business and Finance Office.
4.1.9 The Resident shall be required to enter the premises using the Main Door to the Reception area, adjacent to the Hall Manager.
4.2 Termination
4.2.1 Expiration of the Contract of Lease.
4.2.2 Graduation, Transfer, and Expulsion from the College of Nursing, Foundation University at any time even before the end or expiration of the contract in which case, no refund is allowed.
4.2.3 Death of the Resident.
4.2.4 Before checking out, resident shall allow the Management to check the room and conduct inventory of accountabilities assigned in his/her room.
4.2.5 Upon termination due to reasons stated in the preceding paragraphs, Hall Clearance, Exit Pass, and Exit interview must be secured by the resident, Otherwise, no Academic Clearance from Foundation University can be secured.
4.2.6 The Resident Manager and/or Night Steward or Matron, whoever is available, shall do exit Interview.
4.2.7 All fees and charges due to Smeets Hall shall be settled with the Business & Finance Office of Foundation University before a Hall Clearance is issued.
4.3 Key & Room Management
4.3.1 Each resident shall be furnished with his/her own room key.
4.3.2 Residents shall keep and safeguard his / her key at all times.
4.3.3 In case of a lost key, resident should inform the Front Desk at once so preventive action could be taken. The resident shall execute Affidavit of Loss duly notarized to protect him/her from further damages arising out of the lost key.
4.3.4 The Management shall charge the resident FIFTY (PHP50.00) pesos for key duplication expenses.
4.3.5 Before leaving the Hall, residents are required to close the door and double check key, if with them.
4.3.6 Any maintenance required must be reported at the Front Office, properly documented using appropriate form, and signed by the resident.
4.3.7 Absolutely, no posting of stickers/ decals that can deface the walls is allowed.
4.4 Operational Rules
4.4.1 Residents are expected to abide by the provisions of this Manual on Rules and Regulations of Smeets Hall.
4.4.2 Nobody is allowed to enter Smeets Hall without the required Application form from the Office of the Resident Manager, Auxiliary Services, Foundation University or the Business and Finance Office.
4.4.3 Residents are expected to take good care of the leased premises including the furniture, fixtures, and equipment entrusted to them as well as the properties of third persons in the unit.
4.4.4 Alcoholic beverages, cigarettes, illegal drugs, pornographic materials, deadly weapons, matches/ lighters and flat irons are absolutely not allowed in the hall.
4.4.5 When not in use, must be switched off all lights, electric fans, air-con units, exhaust fans, and other electrical appliances.
4.4.6 All faucets must be turned off after every use.
4.4.7 Residents must leave area clean and livable; otherwise, a professional housekeeping fee will be charged.
4.4.8 Residents are expected to maintain their own room neat and clean. Hanging of any personal things is not allowed.
4.5 Security Rules
4.5.1 No Resident is allowed entry to or exit from the hall without logging in or out by swiping her/his University Identification Card at the unit located at the Front Desk.
4.5.2 Resident shall allow the Security Guard/ Officer, Front Desk Personnel, Night Steward / Matron, and/or the Manager of the FU Hall Services / Auxiliary Services to inspect his/her bags, pouches, pockets, body and other personal belongings for safety and protection.
4.6 Mess Hall Rules
4.6.1 Should meals be allowed in the Hall, plates and cutleries must be placed in the servers after every meal. Students must push back the chairs under the tables.
4.6.2 Residents are required to clean their mess/ table after eating should meals be allowed in the Student Lounge.
4.6.3 No items assigned in the Student Lounge are to be taken out of the area unless authorized by the Management.
4.7 Waste Disposal Management
4.7.1 Individual waste / trash / garbage is to be placed in the plastic bags provided and shall be deposited in the central floor garbage bin located at the Comfort Lounge of every floor.
4.7.2 Waste / trash / garbage deposited in the Comfort Lounge shall be collected early every morning and late in the evening.
4.7.3 At all times, no waste / trash / garbage shall be collected from the room of each resident.
4.8 Comfort Lounge/ Shower Room
4.8.1 Keep and maintain all comfort lounges in clean, sanitary, and comfortable order at all times.
4.8.2 Do not throw tissue papers, napkins, papers, and other objects in the toilet bowl to avoid clogging. Trash bins are provided in every lounge for proper disposition of waste.
4.8.3 Washing/Hanging of clothes/under wears is not allowed in the comfort lounge nor is it allowed inside the room and cabinet. Residents may wash their underwear at the laundry room.
4.8.4 To avoid longer cue and conflicts, residents are encouraged to take turns.
4.9 Reception Area, Entrance & Exit Points
4.9.1 The Front Office shall be treated as the Reception Area for all transactions including the automated Time-In and Time-Out of Residents and Employees.
4.9.2 Time-In and Time-Out shall be done on the Computer located at the Front Desk.
4.9.3 No entrances and exits shall be allowed in the South Door of the Hall. The South Door is reserved as Exit Point in case/ of emergency only.
4.10 Visitors, Visiting Hours & Weekend Pass
4.10.1 Visitors are allowed only from 8:30 p.m. to 10:30 p.m.
4.10.2 The designated visiting lounge shall be the Reception Lounge only. However, if the Student Lounge is not in use, visitors can be accommodated in the area.
4.10.3 Absolutely, no visitors are allowed to enter the Resident’s room. Residents are strictly obligated to follow this specific rule at all times.
4.10.4 Parents who wish to see the rooms of their children shall obtain clearance from the Office of the Resident Manager first before any pass is allowed.
4.10.5 If going on a weekend leave, Resident must apply for a Weekend Pass with the Office of the Resident Manager at least one (1) day prior to his/her leave by completely filling out the Hall Weekend Pass.
4.10.6 Resident on Weekend Leave must require his/her Guardian listed in his Application Form to sign the Weekend Pass Form.
4.10.7 Upon return to the Hall, the Resident must present his/her Weekend Pass to the Front Desk before any entry is allowed.
4.11 Offenses defined: the University and/ or Hall Management by virtue of this Manual on Rules and Regulations generally and absolutely prohibits the offenses enumerated and detailed below.
4.11.1 Hall Attendance, Time Table.
4.11.1.1 Coming in or reporting to the Hall late. Before curfew time, all residents are expected to be in the Hall.
4.11.1.2 The following official Hall Schedule must be strictly observed:
6:00 - 7:20 - Shower time (each floor)
7:00 - 7:30 - Breakfast
7:00 - 12:00 - Academic / Duty Hours
12:00 - 1:00 - Lunch
2:30 – 6:00 - Academic/ Duty Hours
6:00 - 7:30 - Dinner
7:30 - 12:00 midnight STUDY HOURS
10:30 - Curfew
12:00 - Lights Off
4.11.2 Leave Without Official Weekend Pass. Absence from the Hall without officially securing/applying for Weekend Pass and/or without permission, failure to notify or advise the Hall Management of the cause thereof, or even if he/she gives such notice or advice, if in the opinion of the Management.
4.11.3 Misconduct and Misbehavior.
4.11.3.1 Bringing of FOOD, CANDIES, ALCOHOLIC & SWEET BEVERAGES, and PORNOGRAPHIC MATERIALS into the Hall Room or any part of the premises not designated.
4.11.3.2 Bringing/Hiding of illegal/prohibited drugs and deadly weapons in any part of the Hall.
4.11.3.3 Disturbing Resident or Officer/Personnel by way of unnecessary sounds or noise that can disrupt rest and/or academic hours.
4.11.3.4 Having the volume of the music being so loud that it can distract other resident’s/or any personnel from the Hall Management during rest and/or academic hours.
4.11.3.5 Not strictly observing the Dress Code prescribed by the University / Hall Management or wearing indecent clothes that may disturb or distract Residents.
4.11.3.6 Unjustified failure to wear identification cards when provided and required by the University / Hall Management especially when entering the Hall premises.
4.11.3.7 Unjustified failure to login or log-out either electronically or manually whichever is required by the University / Hall Management.
4.11.3.8 Unauthorized use of office landlines, mobile phones, Internet, & other means of communication for personal calls, and unauthorized or disallowed receipt of personal calls.
4.11.3.9 Loitering in the Hall Lobby or lobbies, Hall Management Office, Front Desk, Work Site, Cafeteria, Comfort Lounge, or Pantry during Academic Hours, Study Times, Lecture/Meeting/Counseling Time, or while on Weekend Leave.
4.11.3.10 Taking part in any bookmaking, gambling, lottery, or any other game of chance inside the Hall premises.
4.11.3.11 Holding any unauthorized meeting, gatherings, or assembly within the Hall premises that would adversely affect Hall operations / activities.
4.11.3.12 Fighting or attempting bodily harm on any person except in self-defense while within Hall premises or while performing work whether within or without Hall premises.
4.11.3.13 Commission of an act that may constitute a crime within or without Hall premises including, but not limited to, drug trafficking, drug addiction, illegal possession of firearms, etc.
4.11.3.14 Commission of an immoral act within or outside Hall premises.
4.11.3.15 Intimidation or coercion of fellow residents and other parties that adversely affects Hall/Management interest.
4.11.3.16 Concealing a disease that endangers fellow residents.
4.11.3.17 Refusal to submit to, or failure to meet, security requirements of the Hall Management or the University.
4.11.3.18 Carrying prohibited or deadly weapons or banned objects within Hall premises.
4.11.3.19 Malicious mischief or horseplay resulting in injury to persons or destruction of Hall property or any other property for which the Management may be held liable.
4.11.3.20 Intentionally destroying or damaging Hall property or any property for which the Management may be held liable, including electrical/computer equipment and/or devices.
4.11.3.21 Circulating false, untrue or confidential information affecting the interests of the Hall Management, its residents, officers, clients and other employees.
4.11.3.22 Swearing and use of disrespectful language.
4.11.3.23 Habitual failure to secure a Weekend Pass.
4.11.3.24 Being established to be a poor security risk.
4.11.4 Dishonesty.
4.11.4.1 Unauthorized use of Hall resources.
4.11.4.2 Stealing or attempting to steal from the Hall, its employees, fellow residents, customers and/or clients.
4.11.4.3 Obtaining or attempting to obtain Hall funds, equipment, product, supplies and materials through fraudulent means from the Hall suppliers, warehouses, facility, or other work places.
4.11.4.4 Falsifying personal or other records, either upon applying for hall service or during stay.
4.11.4.5 Offering or receiving money or other valuable consideration in exchange for an unwanted service, better dwelling place or any change in dwelling conditions.
4.11.4.6 Substituting Hall materials and / or items with the intent to gain.
4.11.4.7 Deliberate or willful submission of wrong personal information / document for the purpose of claiming over or counting over Time Record.
4.11.4.8 Falsifying personal TIME-IN and TIME-OUT records either deliberately doing it personally or through an agent or another resident.
4.11.4.9 Unauthorized viewing or watching of videos / movies during academic hours and/or within the Hall premises at any time, especially those which are not related to the student’s academic discipline or not allowed by the Management.
4.11.5 Insubordination/ Discourtesy.
4.11.5.1 Failure to carry out instructions of his / her superior, level coordinator/ floor leader, or Hall Management.
4.11.5.2 Serious insult or willful disrespect to any Hall official or employee or persons transacting business with the Management within Hall premises.
4.11.5.3 Refusal of a resident to answer questions in any investigation authorized or conducted by the Management, unless such answers would violate his/her constitutional rights.
4.11.5.4 Disobedience to the lawful orders of his superiors in connection with his stay in the hall resulting in disruption of Hall operations / activities.
4.11.6 Negligence of Personal Duty.
4.11.6.1 Failure to log IN or OUT either through electronic or manual form.
4.11.6.2 Failure to observe, follow, and carry out Hall Rules and Policies explicitly laid down for Hall Residents or Rules and Policies which may be given by the immediate Supervisor or Management from time to time.
4.11.6.3 Wasting time or loitering in comfort lounges, lobbies, canteen, medical clinic, other resident units, or any other place away from his leased place while on board.
4.11.6.4 Frequently receiving visitors during academic hours for personal business or receiving or making frequent personal telephone calls.
4.11.6.5 After having access to information, failure to report, as soon as practicable, loss, spillage, or damage to Hall property or any other property or product for which the Hall Management may be held liable.
4.11.7 Taking Alcoholic Beverages and/or Prohibited Drugs.
4.11.7.1 Drinking liquor within Hall premises, except on occasions and in locations authorized by the Management.
4.11.7.2 Reporting to the Hall while under the influence of liquor and/or prohibited drugs.
4.11.7.3 Unauthorized use or possession of prohibited / regulated drugs and/or liquor within Hall premises.
4.11.7.4 Selling or inducing any person to take prohibited / regulated drugs and/or liquor within Hall premises, except when duly authorized for medical reasons.
4.11.8 Violation of Safety Rules.
4.11.8.1 Unauthorized use of any Hall property especially when not assigned to such privilege to use or when on an approved Weekend Leave.
4.11.8.2 Unauthorized use of fire fighting equipment for purposes other than fighting fire.
4.11.8.3 Tampering or removing safety devices from machinery, personal computers, and other equipment, electronic gadgets, or any other property without permission.
4.11.8.4 Allowing unauthorized persons and/or co-residents to operate or use Hall equipment, machines either assigned to the offender or for office use.
4.11.8.5 Having open flames in prohibited areas within Hall premises / property.
4.11.8.6 Smoking in “No Smoking” areas within Hall premises or property.
4.11.8.7 Failure to observe other Hall safety rules and regulations.
4.11.8.8 Unauthorized entry and exit through the South Door designated as Emergency Exit Point only or through any other door not designated as entry and exit points.
4.11.8.9 Unauthorized entry in “Restricted Areas.”
4.11.9 Violations of Security Rules.
4.11.9.1 Failure of the Resident to submit him/herself for security checks before coming in or leaving the Hall premises, or in any other area/s designated by the Management for official use.
4.11.9.2 Failure of the Resident to submit his/her personal belongings including hand bags, lady’s bags, pouches, and other similar items for security check before coming in or leaving the Hall premises or in any other area/s designated by the Management for official use.
4.11.9.3 Failure of the Resident to leave his/her personal belongings, including but not limited to Mobile Phones, Cameras and Electronic Gadgets unattended.
4.11.9.4 Failure to observe Security Rules & Regulations.
4.11.10 Violation of any Provision of the Contract of Lease.
4.11.10.1 For this offense, the Contract Agreement shall be enforced.
4.12 Disciplinary Actions
4.12.1 Written Warning. This is a written notice given a Resident for having committed an infraction of a rule, calling his attention that repetition of the offense will be dealt with more severely. The Dean of the College of Nursing, the Office of the President, the Office of the Vice President for Academic Affairs, and the Office of Student Life & External Affairs shall be furnished with this notice.
4.12.2 Written Reprimand. This is a written admonition to a Resident who commits the same or another offense of the same degree during the period of observation after receiving a written warning, or a more serious offense whether he has received a written warning or not. The degree of an offense is determined by the corresponding penalties imposed.
The same Notice shall be furnished to the Offices of the Dean of the College of Nursing, Vice President for Academic Affairs, and Vice President for Student Life & External Affairs.
4.12.3 Monetary Penalty. A fine of PESOS: TWO THOUSAND (PHP2, 000.00) pesos shall be imposed on a Resident who commits the same infraction or infractions of the degree after the Written Warning and Written Reprimand have been served.
4.12.4 Summary Dismissal. This is termination of a Resident for a cause or commission of an extremely serious violation of Hall Rules or Regulations on Discipline.
Summary Dismissal deprives a Resident of his rental during the period of his/her dismissal from the property in which case, no refund on rental already computed and included in his/her Personal Account shall be given.
4.12.5 Qualified Penalty. The penalties of Written Warning, Written Reprimand, and Monetary Penalty may be qualified by the extent or amount of the loss or damage or injury to persons, the gravity or seriousness of the offense and / or circumstances of its commission, in which case such penalties could extend up to Summary Dismissal.
4.13 Forms To Be Used
4.13.1 Disciplinary Action Report (Form DM-100)
4.13.2 Notice of Disciplinary Action (Form DM-101)
4.14 Implementation. Immediately after the Resident allegedly commits an offense, Administrators shall:
4.14.1 Investigate and review the facts of the case, giving Resident concerned opportunity to be heard and present his side.
4.14.2 Accomplish Form DM 100 (Disciplinary Action Report).
4.14.3 Determine the appropriate penalty to be imposed, in consultation with the Resident Manager.
4.14.4 Advice Resident concerned of penalty enforced by accomplishing Form EM 101, “Notice of Disciplinary Action” and serving such notice personally or by registered mail.
